Frequently Asked Questions about Vaccination Laws

Below is a list of additional documents and information that are required to complete enrollment. You can upload these documents into the Aeries system during initial enrollment or as determined when contacted by the ASJUSD Office Staff.

1. Enrollment Confirmation from Aeries - this is available to print once you have completed the online enrollment application. If you do not have access to a printer, you can print to PDF or ask our office staff to print this for you.
2. Report of Health Examination for School Entry Form by 1st Grade. This can be completed with 18 months prior to the start of 1st grade OR families can submit the Waiver of Health Examination Form.
3. Kindergarten: Oral Health Assessment Form or Waiver Request Form.
4. Current IEP or 504 Plan (if applicable)
5. Names and contact information for at least three (3) emergency contacts.
6. Previous school's name(s), address(s), and district information for the prior two (2) years.
7. An academic transcript or report card from your previous school.
8. Proof of withdrawal from your previous school district.
9. Court Custody, Restraining Orders or other papers (if applicable).
10. For EL (English Learner) students, a copy of current ELPAC scores.

All forms and information can be uploaded through the Aeries Online Portal or brought to the school office to complete enrollment.

Families requesting an interdistrict or intradistrict transfer for the upcoming school year must complete the appropriate transfer request in addition to the online enrollment process. Transfer requests are subject to district approval and space availability.

Transfer Information and Requests

The Aromas-San Juan Unified School District is diligent in assuring that all students who attend our schools live within the boundaries of the school district or hold a valid Interdistrict transfer. Residency is based on where you and your child(ren) live, not the property you own. State law requires that public schools verify student residency within the school district (CA Education Code 48204.1). Every family enrolled in the Aromas-San Juan Unified School District is required to provide residency verification as outlined below upon initial registration.

You must provide the following documentation. The items must have your name and physical address where the student sleeps at night. The requirements can also be downloaded for reference.

1. A Current CA Driver's License, ID Card, or other Government-Issued Identification. This is for identification purposes only.
    
2. ONE of the following documents as outlined below:

   • Current Rental/Lease agreement  which includes the following:  Parent Guardian AND Student name(s), Verification of occupancy for current school year, Manager/Owner name and contact information and signatures of both parties

   • Current property tax bill

   • Current Homeowner’s or Renter’s Insurance Policy or Declaration page

   • Recent Escrow Statement
      

3. TWO documents from the following list:

   • Current acceptable utility bill from: PG&E, Water, Garbage, Cable or Internet

   • Any current document from a Federal, State or County Agency (Excluding documents from the DMV)

   • Current Vehicle Registration

   • Recent Sample Ballot (within 3 months)

Residency is based on where you live, not what property you own.  Purchase agreements/contracts are not acceptable as proof of residency (escrow closing papers are acceptable with additional verification required within 30 days).  If you have special circumstances and are unable to provide any of the residency documentation, please communicate this with school staff during the enrollment process.

• Certificated copy of the birth certificate

• Alternate birth record
• Affidavit of birth by parent/guardian
• Current valid passport
